What telehealth means here
Your intake and screening are reviewed remotely by a licensed clinician who authorizes your treatment order. Review may be asynchronous — the clinician evaluates your submitted information without a live call — or may include a synchronous video or telephone visit if clinically indicated.
Benefits and limitations
Telehealth improves access and convenience and lets a clinician oversee care delivered in your home. Its limitations include the absence of a hands-on physical examination, which means some conditions cannot be assessed and your clinician may determine that an in-person evaluation is required before treatment can proceed.
Risks
- Information you submit may be incomplete or inaccurate, which could affect clinical decisions.
- Technology failures or delays may interrupt or postpone your review.
- Despite our safeguards, no electronic transmission can be guaranteed to be completely secure.
- A remote review may miss a finding that an in-person examination would detect.
Your rights
You may withdraw this consent at any time without affecting your right to future care. You may request an in-person evaluation instead. Your telehealth records become part of your medical record and are protected by our Notice of Privacy Practices. Visits are not recorded without your separate written permission.
Licensure and location
Care is provided only where our clinicians and nurses hold an active license. You agree to give us your accurate physical location at the time of service so that the correct licensed professional is assigned and emergency services can reach you if needed.
